ArtsQuest- Win tickets to a Musikfest Cafe concert

ELIGIBILITY: The contest is open only to legal residents of the United States of America at least 13 years of age. If you are under the age of 18 you must obtain parent or legal guardian's consent to enter this contest. The contest is subject to all federal, state and local laws and is void where prohibited or restricted by law. Employees of ArtsQuest, Good Sheperd Rehabilitation Network and their immediate families (spouse, parents, siblings and children and/or individuals living in the same household) are not eligible to enter or win. Winner may not have won a prize from ArtsQuest in the past thirty (30) days.
HOW TO ENTER: Read all terms and conditions. Enter the Musikfest Cafe ticket "contest" by completing entry form at http://www.artsquest.org/contest. By entering, entrant acknowledges and agrees that ArtsQuest. Good Sheperd Rehabilitation Networkand the agents, employees, directors and officers thereof, will have no liability whatsoever for any injuries, losses or damages of any kind incurred in connection with or resulting from possession or use/misuse of any content and/or participating in this promotion or any promotion-related activity. One entry per valid e-mail address will be accepted. The submission period begins Wednesday, January 11, 2012 upon the posting on the contest at www.artsquest.org and ends Thursday, March 1, 2012 at or around 11:59 p.m. EST. ArtsQuest is not responsible for entries that are lost, stolen, late, misdirected, mutilated, tampered with or arrive postage due. All submissions become the property of ArtsQuest and will not be returned.
ENTRY DEADLINE: To be eligible to win completed entries must be received online by Thursday, March 1, 2012 at or around 11:59 p.m. EST or send a self addressed stamped postcard with name, address, city, state and phone number and email to ArtsQuest, 25 W. Third St., Bethlehem, PA 18015 by Thursday, March 1, 2012.
SELECTION OF WINNER:
Grand Prize Winner: One (1) entrant, selected at random, will be chosen as winner from among all eligible entrants that have successfully completed an entry form.
The decision of ArtsQuest will be final in all matters pertaining to the selection of the winner. Odds of winning are dependant on the number of eligible entries received. If the winner(s) cannot be reached at the telephone number supplied in his or her contest entry after reasonable efforts are made to call, that winner(s) will be disqualified and ArtsQuest will select another entry at random.
DESCRIPTION OF PRIZE(s): Grand prize: Winner will receive a pair (2 tickets) of tickets to a concert of their choice at the Musikfest Cafe, ArtsQuest Center at SteelStacks, 101 Founders Way, Bethlehem, PA. Prize is subject to availability. Approximate retail value is $100. Taxes on the prizes are the sole responsibility of winner. The winner(s) must claim their prize(s) in person during normal ArtsQuest business hours within five (5) days of notification. Prizes other than cash are not redeemable for cash. Prize(s) are nontransferable, non-negotiable and no substitution is allowed, except by ArtsQuest or contest sponsors who reserve the right to award a prize of equal or greater value if advertised prize is unavailable. Winners are responsible for any necessary transportation, lodging, meals and other accommodations associated with the prize(s). The prize will be forfeited if the selected winner (a) does not claim the prize in person (ArtsQuest may require proof of identity) within the required time, (b) is not eligible in accordance with these rules, or (c) does not execute the prepared release and waiver described below. If the prize is forfeited for the reasons set forth above, no substitute winner will be selected.
GENERAL CONDITIONS: Entrants agree to be bound by the terms of these official rules. The laws of the United States govern this contest. All federal, state and local laws and regulations apply. Winners may be required to execute and return an Affidavit of Eligibility and Liability and where legal, Publicity Release within 14 days following the date of issuance of notification. Non-compliance within this time period or return of any prize/prize notification as undeliverable may result in disqualification and selection of an alternate winner. By entering, participants release and hold harmless ArtsQuest and its respective subsidiaries, affiliates, directors, officers, prize suppliers, employees and agents from any injuries, loss, damage or liability of any kind arising from or in conjunction with this contest or acceptance or use of any prize won. You may opt out at any time.
ADDITIONAL TERMS: ONLINE SUBMISSION: ArtsQuest is not responsible for entries not received due to lost, failed, delayed or interrupted connections, including Internet/Web-server/network/phone-line connections, or other electronic malfunctions, computer/telephone hardware or software failures/malfunctions. ArtsQuest reserves the right, in their sole discretion, to cancel or suspend this contest should virus, unauthorized human intervention, or other causes beyond the control of ArtsQuest that, in the sole opinion of ArtsQuest, corrupt the administration, fairness, or proper operation or play in the contest. Any attempt to damage or undermine the fair and legitimate operation of this contest will result in disqualification.
LIABILITY: By entering the contest, entrant agrees that ArtsQuest, Good Sheperd Rehabilitation Network and the agents, employees, directors and officers thereof, will have no liability whatsoever for any injuries, losses or damages of any kind incurred in connection with or resulting from participation in this promotion, or while preparing for and/or participating for this promotion or any promotion-related activity. By accepting a prize, winner agrees that ArtsQuest, Good Sheperd Rehabilitation Network and the agents, employees, directors and officers thereof, will have no liability whatsoever for any injuries, losses or damages of any kind incurred in connection with or resulting from acceptance, possession or use/misuse of any prize, or while traveling to, preparing for and/or participating in this promotion or any prize-related activity.
PUBLICITY: By acceptance of a prize, winners consent to the use of their name, photographs, voice or other likenesses, testimonials and other biographical information for any and all purposes, including but not limited to advertising, trade and/or promotions on behalf of ArtsQuest without further compensation, unless prohibited by law.
